A Primary Care Provider (PCP) is a physician or advanced care practitioner, such as a nurse practitioner or physician’s assistant, responsible for monitoring your overall health and coordinating your care. PCPs provide comprehensive services, including chronic, preventive, and acute care, while developing long-term relationships with their patients. Our family medicine specialists can address most of your medical concerns and facilitate referrals to specialists when necessary.

You will receive a text message to confirm your appointment. After you confirm your appointment you will receive a text with a link to complete new patient paperwork. Please complete that paperwork online if you are able to in advance to save you time on the day of your visit. During this visit, your provider will review your medical history and medications and address one or two health concerns. Please note that preventive annual exams are not performed during the first visit unless you are healthy and not taking any medications.
An annual physical or routine preventive exam is essential for individuals of all ages. The goal is to detect health issues early before they become serious. It is recommended that you have this exam once a year. The preventive exam includes screenings for cancer, cholesterol, STIs, blood pressure, diabetes, and kidney disease, as well as vaccinations and discussions on healthy lifestyle choices.
Most insurances waive copays and deductibles for routine annual physical exams, but specific health issues outside the scope of the preventive exam may incur charges that apply to your copay/deductible.

The Medicare Annual Wellness Visit differs from a standard annual physical exam. During the wellness visit, your provider will review your past medical history and family health history and discuss potential treatment options if you show signs of certain conditions. You should bring a list of your current medications, or photos/bottles, to share with your provider.
Your physician may also perform screening tests, such as cognitive health screenings to detect early signs of memory loss or dementia, discuss advanced directive, screen for risk for falls, and a perform a depression assessment to better understand your mental health.
A medicare wellness exam is required every year for patients age 65 and older. The annual wellness exam does not include a physical exam or labs.

If you need to cancel or reschedule, please call our office at least 24 hours in advance to avoid any cancellation fees. You may also respond to the automatic appointment reminder text indicating you would like to cancel or reschedule your appointment.

While we do not accept walk-ins, we often have same-day or next-day openings for urgent issues. Please call us to check availability.
Yes, you may bring one adult guest to your appointment. Friends and family can provide valuable support and information regarding your health. However, we will also give you the opportunity to speak privately with your provider if there are topics you prefer to discuss alone. If you bring children, please note that you are responsible for their care during your visit, as we do not have childcare services available.
We recommend an annual visit to maintain your health, even if you do not have any specific concerns. If you are on routine medications or have chronic health conditions, regular visits are necessary for lab checks, monitoring, and medication refills.
We collaborate with nearly all local hospitals, allowing you to choose where you’d like to receive care. We can send your medical records to the hospital and coordinate with their staff to ensure seamless care. After a hospital visit, we recommend scheduling a follow-up appointment with us to review your records, check on your condition, update medications, and provide any necessary referrals to specialists.
For prescription refills, please contact your pharmacy directly. If physician approval is needed, they will reach out to us. Please allow up to 72 hours for the processing of refill requests, and note that these requests will not be processed after hours.
We offer urgent care coverage for our patients at all times. If you have an urgent issue, please call our main office number, and you will be directed to our on-call provider. For immediate emergency care, please call 911. Non-urgent questions and medication refills cannot be addressed through the on-call provider but you may text the office through the chat in the bottom right corner of your screen or your phone and we will respond within 1 business day.
For routine appointments, we recommend arriving 15 minutes early. If you are a new patient, please arrive 30 minutes prior to your appointment to allow time for paperwork.
Yes, you can choose to have your appointment in-person or via our telemedicine program if your provider determines it’s appropriate for your situation. Telemedicine visits can be conducted through your phone or computer and include both video and audio. Please note that new patients must have their initial visit in the clinic, and preventive annual exams cannot be conducted via telehealth.
Insurance plays a significant role in our billing process. Before your appointment, please verify that we accept your insurance to avoid unexpected charges. Healthy Futures accepts most major insurances, so it's also important to contact your insurance provider to assign us as your primary care provider.
Remember to bring your insurance card and ID to your appointment for proper processing.
We accept cash, personal checks, Visa, MasterCard, AMEX, and Apple Pay. You can also make secure online payments through our website.
